To Register for the Man Up Program, complete the information requested below.
In the box marked "Message" below, enter the following additional required information to complete your registration:
The Date Young Person and Parent/Guardian are planning to begin the program.
Young Person's Name
Location (Carrollton, TX, Denton, TX, or Tulsa, OK) at whch you are planning to attend.
The Name of the Parent/Guardian who is planning to attend the program. (Young person cannot attend without Parent/Guardian present at all times.)
Phone number at which Parent/Guardian can be reached.
